IMPORTANT UPDATE
The old Request Management System (RMS) is shut down permanently.
To confirm if you have a grant from the old system, the grant ID numbers are letters plus 5 numbers: MED-TD-27322, PED-HD-27302. If you are trying to
submit a reconciliation in the old system, please email it to tevarequestmanagement@tevapharm.com. Please show where the Teva funds were used and how much vs. what was submitted and please include outcomes/metrics and an Executive Summary.
To submit a grant request you must first be registered in the new RMS. Using Google Chrome or Firefox, go here to register: https://webportalapp.com/sp/login/teva-registration.
Once your registration is approved, you will receive an email from the RMS with the submission link (https://webportalapp.com/sp/login/teva-grantrequest) and you will be able to submit a request.
